超敏cTnT、超敏cTnI、CK-MB质量和CK-MB活性诊断AMI的灵敏度、特异度以及准确度分析
Sensitivity, specificity and accuracy of high-sensitive cTnT, high-sensitive cTnI, CK-MB mass and activity for acute myocardial infarction

Abstract
Objective High-sensitive troponin T (cTnT),high-sensitive troponin I (cTnI),creatine kinase isoenzyme (CK-MB) mass and activity were measured,analyzed the sensitivity,specificity and accuracy of these indicators in the diagnosis of acute myocardial infarction (AMI),which provided proper diagnosis marker of AMI.Methods The levels of serum high sensitive cTnT,cTnI and CK-MB from AMI group,stable angina pectoris (SAP) group and healthy control group were detected by electrochemical luminescence method,chemiluminescence method and turbidimetry respectively at different blood sampling time points.Then the results were statistically analyzed,and the sensitivity,specificity and accuracy were calculated.Results At different blood sampling time points (0 h,3 h and 6 h),the sensitivity of high-sensitive cTnT in the diagnosis of AMI was 62.89％,95.88％,100％ respectively,the specificity was 98.76％,98.76％,98.76％ respectively,and the accuracy was 79.2 1％,97.19％,99.44％ respectively;the sensitivity of high-sensitive cTnI in the diagnosis of AMI was 72.16％,96.91％,98.76％ respectively,the specificity was 98.76％,97.53％,97.53％ respectively,and the accuracy was 84.27％,97.19％,98.31％ respectively;the sensitivity of CK-MB mass in the diagnosis of AMI was 27.00％,63.92％,92.78％ respectively,the specificity was 92.59％,90.12％,88.89％ respectively,and the accuracy was 55.62％,75.84％,91.01％ respectively;the sensitivity of CK-MB activity in the diagnosis of AMI was 28.86％,71.13％,93.81％ respectively,the specificity was 87.65％,83.95％,81.48％ respectively,and the accuracy was 55.62％,76.97％,55.62％ respectively.Conclusion The sensitivity,specificity and accuracy of high-sensitive cTn in the diagnosis of AMI were significantly higher than those of CK-MB mass and CK-MB activity.Hypersensitivity cTn is applied to the diagnosis and differential diagnosis of AMI,and can greatly improve early diagnosis of AMI.关键词
