18F-NaF PET/CT对初诊肺癌患者骨转移的价值
Value of 18F-NaF PET/CT in detection of bone metastasis in initial lung cancer

Abstract
ObjectiveTo compare the value of preoperative 99mTc-MDP bone scintigraphy (BS) and 18F-NaF PET/CT in diagnosis of bone metastasis in patients with initial lung cancer. Methods: 49 patients with recently diagnosed lung cancer were examined with 18F-NaF PET/CT and BS within 1 week. The diagnosed results of both imaging modalities were compared at the level of patient and lesion for receiver operating characteristic curve(ROC) analysis. The area under the ROC curve represented the efficacy of diagnosis system. ResultsAt the patient level, 27 of the 49 patients were proved to have bone metastasis. 18F-NaF PET/CT correctly diagnosed all 27 patients with bone metastasis, while BS correctly detected 24 of them (88.9%). 18F-NaF PET/CT had no false diagnosis. While BS produced 4 false-positives and 3 false-negatives. At the lesionlevel, final diagnosis was obtained in 138 lesions, including 100 metastatic lesions and 38 benign lesions. 18F-NaF PET/CT correctly diagnosed 100 metastatic lesions and 36 benign lesions, with 2 false-positive lesions. BS diagnosed 62(62%)metastatic lesions and 15 benign lesions, with 6 false-positive and 2 false-negative, and the rest lesions were missed. For 18F-NaF PET/CT, the area under the ROC curve was 1 at patient level(for BS it was 0.854, P<0.05)and 0.974 at lesionlevel (for BS it was 0.731, P< 0.05). Conclusion The diagnostic accuracy of 18F-NaF PET/CT on bone metastasis in initial lung cancer patients is higher than that of BS.关键词
