Abstract
Objective To investigate the efficacy and adverse reactions of sildenafil combined with high-frequency oscillatory ventilation ( HFOV) on persistent pulmonary hypertension of newborn ( PPHN) .Methods From February 2014 to February 2015 60 neonates with PPHN hospitalized in pediatrics department of Affiliated Hospital of Medical College of Ningbo University were divided into control group and observation group randomly with 30 cases in each.Pulmonary arterial pressure (SPAP), systemic blood pressure (SBP), arterial oxygen partial pressure ( PaO2 ) , arterial blood oxidation carbon partial pressure ( PaCO2 ) , inhalation of oxygen ( FiO2 ) and arterial oxygen saturation ( SaO2 ) before and after treatment were monitored for all patients.The control group was treated with HFOV, while the observation group was treated with sildenafil combined with HFOV.Arterial blood gas and pulmonary artery pressure before and 3 days after treatment, and adverse reactions were monitored.Results After treatment, PaO2 and SaO2 of two groups were significantly higher than those before treatment ( t value of control group was 2.011 and 2.241, respectively, t value of observation group was 2.939 and 2.839, respectively, all P<0.05), while PaCO2 and FiO2 were significantly lower compared with those before treatment (t value of control group was 2.193 and 2.388, respectively, t value of observation group was 2.920 and 2.933, respectively, all P<0.05).After treatment PaO2 and SaO2 in the observation group were significantly higher than those in the control group ( t value was 2.780 and 2.121, respectively, both P<0.05), while PaCO2 and FiO2 were significantly lower than the control group (t value was 2.430 and 2.939, respectively, both P<0.05).After treatment SPAP in the observation group was significantly lower than the control group (t=2.439,P<0.05), but neonates circulation was not significantly different in systolic blood pressure between two groups (t=0.321,P>0.05).Total effective rate of the observation group (96.7%) was significantly higher than that (83.3%) in the control group (χ2 =4.403,P<0.05).Conclusion Sildenafil combined with HFOV is a more safe and effective clinical treatment for PPHN, and it is worthy of clinical application.关键词
