亚临床甲状腺功能减退症与糖尿病肾脏疾病的相关性
Correlation between the subclinical hypothyroidism and the type 2 diabetes patients with diabetic kidney disease

Abstract
Objective To investigate the correlation between the subclinical hypothyroidism (SCH) and the type 2 diabetes patients with diabetic kidney disease (DKD). Methods Four hundred and four patients of type 2 diabetes with DKD were enrolled in this study. According to the levels of Thyroid-stimulating hormone (TSH)、 Free Triiodothyronine (FT3)、 free thyroxine (FT4), the patients were divided into two groups as follows, the normal thyroid function group (331 cases) and the SCH group (73 cases). By comparing the difference of clinical indicators and related complications between these two groups , the relevance between SCH and DKD was analyzed. Results The SCH group had higher serum creatinine (P<0.001) and blood urea nitrogen (P< 0.001), lower proportion of estimated glomerular filtration rate (P < 0.001) than those in the normal thyroid function group. The proportion of renal insufficiency [eGFR<60 mL/(min·1.73 m2)] in the SCH group was significantly higher than that in the normal thyroid function group (47.95%vs. 22.96%, P<0.001). eGFR was used as a dependent variable for the logistic regression analysis, SCH was the independent risk factor for renal insufficiency in patients with DKD (OR: 2.015, 95%CI: 1.090-3.723, P = 0.025). Conclusion The patients with both DKD and SCH have worse renal impairment than the patients with normal thyroid function, and have higher risk in renal insufficiency; SCH may be an independent risk factor for renal insufficiency in patients with DKD.关键词
