摘要
Abstract
OBJECTIVE:To probe into the entry point of clinical pharmacists in providing pharmaceutical care on chronic cardic failure patients combined with characteristics of various diseases. METHODS:The clinical pharmacists participated in the whole clinical treatment of one case with typical chronic cardic failure, and put forward suggestions mainly focus on the selection of drugs, drug interaction and adverse reaction control etc. Individualized dosing regimen was established, and the intervention results was monitored and assessed. RESULTS: The pharmaceutical care implemented by clinical pharmacists can improve the therapeutic effect of drugs, and effectively avoid the adverse drug interactions, prevent the occurrence of adverse drug reactions. CONCLUSIONS:The clinical pharmacists should go to the clinic to do the deeply research, improve the professional level and basic service skills, provide pharmaceutical care and ensure the rational utilization of drugs in clinic.关键词
