中青年2型糖尿病出院患者多形式随访的效果观察
Effect of Multiform Follow-up on Treatment Compliance and Glycemic Control of Discharged Young and Middle-aged Patients with type 2 Diabetes

Abstract
Objective To observe and analyze the effect of multi-form follow-up management on treatment compliance and glycemic control of discharged young and middle-aged patients with type 2 diabetes. Methods Ninty-eight participants were recruited from June 2013 to June in 2014. Multi-form follow-up such as telephone, short message and QQ network platform was conducted for six months. Patients’ treatment complliance and glycemic control at the end of one month, three months and six months were compared. Results The compliance of diet, exercise and SMBG (self-monitoring of bloodglucose) was significant different ( P<0.01) while the compliance in medicine showed no significant difference in three time-points. At the end of one month after discharge, the compliance of diet, exercise and SMBG were better that that at the end of three months after discharge ( P<0.01). At the end of six months after discharge, the compliance of SMBG was better than that at the end of three months after discharge ( P<0.01). 2hPPG (2-hour postprandial plasma glucose) control at the end of one month and six months after discharge was better than that at the end of three months after discharge (P<0.01). Conclusion Multiform follow-up management can provide all-round and effective management for young and middle-aged patients with type 2 diabetes.关键词
