磁共振扩散加权成像在腹部淋巴结良恶性诊断中的应用价值
Value of diffusion-weighted imaging applied to diagnosis of benign and malignant abdominal lymph nodes

Abstract
Objective To explore the value of diffusion-weighted imaging( DWI) applied to differentiating benign from malignant lymph nodes in abdomen.Methods The MR images of 40 patients,all of 139 lymph nodes including 81 benign nodes and 58 malignant nodes,whose features of lymph nodes confirmed by the postoperative pathology or clinical follow-up were retrospectively analyzed.The short axis diameter,internal signal homogeneity and apparent diffusion coefficient( ADC) were compared between benign and malignant lymph nodes,and the diagnostic efficiency of ADC value in the diagnosis of benign and malignant lymph nodes was evaluated.Results The short axis diameter of benign lymph nodes was shorter than that of malignant lymph nodes,but they showed no statistically significant difference ((8.4 ±2.2)mm vs.(9.1 ±2.2)mm,P>0.05).Fifty-two(64.20%) lymph nodes of signal homogeneity and 29(35.80%) of signal heterogeneity were observed in the benign lymph nodes,while 31(53.45%) of signal homogeneity and 27(46.55%) of signal heterogeneity in the malignant lymph nodes.There was no statistically significant difference in the signal homogeneity degree between benign and malignant lymph nodes(P>0.05).The ADC value of benign lymph nodes was significantly higher than that of malignant lymph nodes((1.4 ±0.24) ×10-3 mm2/s vs. (1.0 ±0.3) ×10 -3 mm2/s,P<0.05).The maximum area under the curve(AUC) of ADC value was 0.879,with optimum threshold of 1 006.50 ×10 -3 mm2/s for the diagnosis of benign and malignant lymph nodes,while the sensitivity and specificity were 100.0% and 75.0%,respectively.Conclusion DWI could be used as an effective technique in the differential diagnosis of benign and malignant abdominal lymph nodes.关键词
