非体外循环冠状动脉旁路移植术后急性肾损伤发生情况及肾功能变化的临床研究
Clinical study on postoperative occurrence of acute kidney injury and changes in renal function after off-pump coronary artery bypass grafting

Abstract
Objective To study the changes in renal function and the occurrence of acute kidney injury(AKI) after off-pump coronary artery bypass grafting ( OPCABG) .Methods A retrospective analysis of 200 patients undergoing OPCABG was conducted .No chronic renal insufficiency was found in any patients before operation .These patients were divided into simplex group (98 cases of coronary heart disease without other diseases before operation ) and complication group (102 cases complicated with hypertension and type 2 diabetes mellitus). Before operation,and 6,24 and 48 hours after operation,the serum creatinine(Scr) and creatinine clearance rate(Ccr) were detected in two groups,and the incidence of AKI after OPCABG was recorded .Results AKI occurred in 23 patients after operation .The incidences of AKI of complication group and simplex group were 14.8%(16/102) and 7.1%(7/98),respectively,and there was no significant difference between two groups(P>0.05).Eleven of 23 patients with AKI needed the hemodialysis(HD) treatment.In two groups,there were significant differences in the levels of Scr and Ccr among different time points(P<0.05).For two groups,the level of Scr started to increase 6 hours after operation and increased with the increasing time,but the level of Ccr started to decrease 24 hours after operation.Six,24 and 48 hours after operation, the levels of Scr in the complicated group were higher than the levels in the simplex group (P<0.05),but the levels of Ccr in the complicated group were lower than the levels in the simplex group (P<0.05).Conclusion Patients without preoperative chronic renal insufficiency also have the risk of suffering from AKI after OPCABG .The complications of hypertension,type 2 diabetes and hyperuricemia have a great influence on the postoperative levels of Scr and Ccr .关键词
