Abstract
It introduced the application of Omaha system in the field of chronic diseases extended care,and put forward that the Omaha system could be used to establish standardized chronic disease discharge guidance pro-gram,so as to explore the extended care pathway which met the need of the prevention and control of chronic diseases in China.关键词
