Abstract
Objective To assess the value of laparoscopic total gastrectomy (LTG) and open total gastrectomy (OTG) for the treatment of stomach neoplasm. Methods Publications on comparision between laparoscopic total gastrectomy(LTG) and open total gastrectomy(OTG) in the treatment of stomach neoplasm were col ected, the data from the publications were matched with the LTG group and OTG group,than the operation time ,blood loss,NO.of dissected lymph nodes,first flatus,first diet,hospital stay day,complication were meta-anlyzed. Results Nine reseaches on 1106 cases of gastric cancer were include in this study ,two of which were randomly control ed studies. The results of meta analysis showed that surgery compared with open surgery group, laparoscopic surgery time longer length (WMD:12.13,95%CI:[8.85,15.40,P,<0.001], blood loss less (WMD:-116.01,95%CI:[-131.87,-100.14,P,<0.001] , NO.of dissected lymph nodes less (WMD:-4.64,95%CI:[-5.64,-3.65,P,<0.001], postoperative intestinal recovery exhaust short time(WMD:-0.07,95%CI:[-0.85,-0.69,P,<0.001] eating liquid after a short time (WMD:-0.11,95%CI:[-0.22,-0.01,P,<0.001], and shorter hospital stay(WMD:-1.47,95%CI:[-1.81,-1.13,P,<0.001]; complications between the laparoscopic group and open group have no significant dif erence(OR:0.79;95%CI:[0.56,1.311](P>0.05). Conclusion Laparoscopic total gastrectomy of gastric cancer is more longer than traditional open total gastrectomy, but in the postoperative recovery much bet er than open total gastrectomy.关键词
