摘要
Abstract
Objective To investigate the hospital-Community-Family Integrated diabetes management intervention. Methods:Bao'an District, Shenzhen People's Hospital, 89 cases of patients with diabetes for a period of 1.5 years hospital - Community - Family Integration comprehensive diabetes interventions, including health education and behavioral intervention, psychological intervention and drug intervention, and for ef ect evaluation. Results: Compared with before intervention, after the intervention of risk factors for diabetes awareness degree, health behavior formation rate, regular medication was significantly increased (P<0.01). Conclusions:Hospital-Community-Family Diabetes management model to promote the integration of diabetes-related knowledge, at itudes, behaviors positive change;blood glucose and other important indicators show preliminary results.关键词
