北京市西城区家庭医生式服务签约居民续约意愿及影响因素研究
Residents′Willingness for Renewing the Contract of Family Doctor Service in Xicheng District of Beijing and Its Influencing Factors

Abstract
Objective To investigate the influencing factors for the willingness of residents for renewing the contract of family doctor service in Xicheng District of Beijing and its influencing factors.Methods Using random sampling method , we selected two community health service centers from Xicheng District of Beijing.We enrolled 721 residents who signed Family Doctor Service Agreement in the two centers from January 2013 to August 2014.Self -designed Family Doctor Service Questionnaire was employed to conduct investigation on the enrolled residents , and the questionnaire content included basic information, the willingness for contract renewal and the awareness , cognition, utilization and demand of the residents toward family doctor service.Results Among the 721 residents , 569 ( 78.9%) residents had the willingness for contract renewal.Significant difference was found in the willingness for contract renewal between the residents who knew about family doctor and the residents who didn′t ( P<0.05); no significant difference was found in the willingness for contract renewal between the residents who knew about family doctor service and contract -signing doctor team and the residents who didn′t ( P>0.05 ) . Concerning the items of cognition , residents with different cognition conditions were significantly different in the willingness for contract renewal ( P <0.05 ) .Significant difference was found in the willingness for contract renewal between residents who received diagnosis and treatment by home visits , health assessment , outpatient diagnosis and treatment , chronic disease management, health consultation and health education and residents who didn′t (P<0.05); no significant difference was found in the willingness for contract renewal between residents who received telephone follow -up, physical examination free of charge , disease rehabilitation , home nursing , appointed referrals , prevention and healthcare services and residents who didn′t ( P>0.05) .Significant difference was found in the willingness for contract renewal between residents who had demand for referrals due to acute/critical/severe diseases , demand for prevention and treatment of infectious diseases , demand for prevention and healthcare services and demand for health education and residents who didn ′t (P<0.05); no significant difference was found in the willingness for contract renewal between residents who had demand for the treatment of common diseases /frequently-occurring diseases, demand for chronic disease management and demand for health education and residents who didn ′t ( P >0.05) .Multivariate stepwise logistic regression analysis showed that whether family doctor service can improve convenience , whether residents accept first diagnosis in communities , whether residents consider family doctor service as important , whether residents received health education and whether residents have demands for acute /critical/severe referral had significant influence on residents′willingness for contract renewal ( P <0.05 ) . Conclusion Residents have great willingness for contract renewal.Whether family doctor service can improve convenience , whether residents accept first diagnosis in communities , whether residents consider family doctor service as important , whether residents received health education and whether residents have demands for acute/critical/severe referral are influencing factors for residents′willingness for contract renewal.关键词
