摘要

Abstract

Objective To observe the influence of dexamethasone on cytokines and glucocorticoid receptor mRNA with the discrepancy of intervening of in ALI rats induced by one-hit and two-hit. Methods 48 male SD rats were randomly divided into 6 groups: control group ( NS ):0. 9% saline was instilled into the trachea and tail vein;one-hit group ( HCl ):HCl ( pH 1. 8, 2 mL/kg ) was instilled into the trachea;two-hit group ( LPS+HCl ):LPS ( 4 mg/kg ) was injected intravenously via a tail vein, 4 hours later, HCl ( pH 1. 8, 0. 5 mL/kg ) was instilled into the trachea;NS+DEX group;HCl+DEX group;LPS+HCl+DEX group. The groups were intraperitoneal injection with dexam-ethasone ( 2 mg/kg ) after replicating model. Results In the groups of one-hit and two-hit, the ( GR ) mRNA was lower than those of the control group ( P < 0. 01 ) . Comparing to corresponding model groups, the GRmRNA in LPS+HCl+DEX groups increased ( P < 0. 05 ) , but not in HCl+DEX groups. In the groups of one-hit and two-hit, the total leukocytes, PMN%, the protein concentration in the BALF and W/D were higher than those of the control group ( P < 0. 05 ) . Comparing with the corresponding model groups, these index in LPS+HCl+DEX groups decreased ( P < 0. 05 ) , but not in HCl+DEX groups. In the groups of one-hit and two-hit, TNF-α, IL-1β, IL-4, IL-10 both in serum and BALF were higher than those of the control group ( P < 0. 05 ) . Comparing to corresponding model groups, TNF-α, IL-1β in LPS+HCl+DEX groups decreased ( P < 0. 05 ) , IL-4, IL-10 in LPS+HCl+DEX groups increased ( P < 0. 05 ) , but not in HCl+DEX groups. Conclusion The pulmonary morphology, the permeability, the inflammatory cytokines and the glucocorticoid recep-tor mRNA in these two models is not consistent. It indicated that pathogenesis of ARDS is different by different risk factors. Dexamethasone can alleviate the pulmonary inflammatory injury caused by"two-hit", but not useful to ALI caused by HCl. So curative effect of glucocor-ticoid is associated with clinical reasons.
