产妇剖宫产术前分娩痛经历对术后静脉镇痛需求量的影响
Influence of pre-operative experience of labor pain on requirements of post-Cesarean intravenous analgesia

Abstract
Objective To examine the effect of the pre-operative experience of labor pain on the requirements of post cesarean section intravenous analgesia. Methods Forty healthy term nul iparous women undergoing Cesarean section using a combined spinal-epidural anesthesia were al ocated to two groups (n=20): selective group and emergency group. Patient-con-trol ed intravenous analgesia(PCIA) technique was administered in both groups after surgery. The visual analog scales(VAS) and the doses of intravenous analgesic of fentanyl required at 12, 24 and 48h postoperatively, the time from end of surgery to the first use of analgesia were recorded. The complications and maternal satisfaction with the post-operative analgesia were observed. Results There were no significant differences in patients characteristics and postoperative VAS scales in both groups (P >0.05). Compared to the selective group, the time to the first use of analgesia was significantly delayed and the consumption of analgesic was significantly reduced in emergency group(P<0.05). In addition, the consumption of analgesic within 48h postoperatively and the time to the first use of analgesia were showed a significant negative correlation with the degree of cervical dilation. There were no significant differences in the incidences of complications and maternal satisfaction(P >0.05). Conclusion Patients who expe-rience labor pain before Cesarean sections have a lower requirement of post-operative intravenous analgesia compared to those not in laboring.关键词
