外科手术后急性肺栓塞23例临床诊治分析

Abstract
Objective To investigate the clinical characteristics, diagnosis and treatment of acute pulmonary embol-ism ( APE) after surgical operation. Methods The clinical data of 23 hospitalized patients with post-surgical APE in our hos-pital during January 2009 and May 2014 were retrospectively analyzed. Results 23 cases of post-surgical APE accounted for 17. 42% (23/132) of all hospitalized patients with APE. The post-surgical APE was often seen in patients with major surgical operation such as general surgery (10 cases), orthopedics (9 cases), tumorous surgical operation (2 cases), gynecologic surgery (1 case) and urologic surgery (1 case), especially the surgery related to malignant tumor (7 cases). 21 cases were confirmed by computer tomography pulmonary angiography ( CTPA) and 2 cases were diagnosed according to clinical features combined with echocardiography, electrocardiogram and arterial blood gas analysis. After anticoagulant and symptomatic treat-ment, 21 patients survived, 2 patients died of sudden hypotension and shock after operation. Conclusion Preoperative risk factors of APE should be performed and early prevention, standard diagnosis and treatment of post-surgical APE are essencial.关键词
