Abstract

Objective To explore the effects of sevoflurane preconditioning and its protection mechanism on pulmonary function in patients with one lung ventilation by thorascopic surgery. Methods There were 60 cases of elderly patients with non-small cell lung cancer age of 50 ~75 years old,weight of 45 ~75 kg and ASA grade Ⅰ ~Ⅱ.All patients were randomly divided into 2 groups with 30 cases each group by digital table,which were sevoflurane pretreatment group (SP group)and control group untreated sevoflurane (P group).At 30 min before anesthesia (T0 ),one -lung ventilation(OLV)30 min (T1 ),OLV 60 min (T2 ), OLV (T3 )and postoperative (T4 )4 mL elbow venous blood samples was collected in order to determine the level of serum malondialdehyde(MDA)and superoxide dismutase (SOD).A piece of normal lung tissue by thorascopic surgery was detected the expression of caspase-3 protein in lung tissue and the concentration of HO-1 from lung tissue homogenate by enzyme-linked immunosorbent assay (ELISA).At the same time,the hemodynamics and relative clinical data were recorded at the five time points. Results The concentration of MDA were significantly lower in SP group (8.74 ±0.78)nmol·mL -1 than that in P group (11.71 ±0.62) nmol·mL -1 at T4 .The concentration of SOD were significantly higher in SP group than that in P group at T3 ,T4 (P <0.05 ).The concentration of HO-1 in SP group (2.22 ±0.23 )ng·mL -1 were significantly higher than that in P group (1 .35 ±0.1 1 )ng·mL -1 (P <0.05).Nevertheless the expression of Caspase-3 in SP group (1 1 .10 ±1 .41 )lower than that in the P group (13.56 ±2.18). Conclusion Sevoflurane preconditioning has protective effects on lung ischemia-reperfusion injury in patients with single lung ventilation in the thorascopic surgery.
