甲氨蝶呤治疗成人血液系统恶性肿瘤的血药浓度与肾功能的相关性研究

Abstract
Objective To analyze the correlation between concentration of high -dose methotrexate ( HD -MTX ) and renal function in adult patients with acute lymphoblastic leukemia ( ALL ) and non -hodgkin lymphoma ( NHL) before and after chemotherapy.Methods A total of 77 hospitalized adult ALL and NHL patients who received 122 times treatment of HD-MTX were retrospectively analyzed.According to 44 h MTX plasma concentration ( C44 h ) , the patients were divided into normal group of 54 cases and the delayed elimination group of 68 cases.Recorded the creatinine clearance (Ccr), cystatin C (Cys C) and C44 h and analyzed the correlation between Ccr , Cys C before and after treatment and C44 h , and compared the changes of Ccr , Cys C and renal toxicity between normal group and delayed elimination group.Results The delayed MTX elimination was 55.7% at 44 h.There was no correlation between Ccr , Cys C and C44 h before treatment ( P>0.05 ).There was a negative correlation between Ccr and C44 h ( P<0.01 ) and a positive correlation between Cys C and C44 h ( P<0.01 ) after treatment.In addition to the Ccr of normal group , there were statistically significant within the group and between groups in Ccr and Cys C before and after treatment.The rate of renal toxicity in delayed elimination group was significantly higher than that in normal group ( P<0.01 ).Conclusion For adult patients with normal renal function , C44 h MTX concen-tration can not be predicted based on the Ccr and Cys C before HD -MTX treatment , but there were significantly corre-lation between the Ccr , Cys C and MTX concentration at 44 h.It suggested that renal function of MTX was still one of the main affected factors of excretion.关键词
