呋塞米联合依那普利对慢性肺源性心脏病心力衰竭的临床研究
Clinical research of furosemide combined with enalapril in the treatment of patients with chronic pulmonary heart disease and heart failure
摘要
Abstract
Objective To investigate the clinical effect of small doses of furosemide combined with enalapril in the treatment of patients with chronic pulmonary heart disease and heart failure.Methods A total of 150 patients of chronic pulmonary heart disease with heart failure were randomly divided into treatment group ( 75 cases ) and control group ( 75 cases).Patients in control group treated by conventional treatment for 2 weeks, while patients in treatment group treated by continuous intravenous infusion of small dose furosemide 2.5-5 mg· h -1 combined with enalapril 10-20 mg· d-1 treatment on the basis of control group.The treatment lasted for 2 weeks.Patients in two groups were assessed according into chronic obstructive pulmonary disease and bronchial asthma physiological score ( CAPS ) before and after treatment , and right ventricular diastolic function , hospital stay time , blood rheology , electrolyte and adverse reactions were recorded in two groups.Results The time of invasive ventilation , intensive care unit ( ICU ) stay , hospital stay in the treatment group were significantly shorter than those in control group , and the difference were statistically significant ( P<0.05 ); CAPS scores after treatment 24 , 48 , 72 h in the treatment group were significantly better than those in the control group(P<0.05).The whole blood viscosity, plasma viacosity after treatment in two groups were significantly lower than those before treatment, while the whole blood viscosity, plasma viacosity in the treatment group were significantly lower than those in the control group ( P <0.05 ).Serum K +, Na +, and Cl - after treatment were significantly lower than those before treatment in the control group , and they in the treatment group were higher than those in the control group after treatment ( P <0.05 ).E peak and Ve after treatment in two groups were significantly higher than those before treatment, A peak and Va after treatment in two groups were significantly lower than those before treatment, but the changes in treatment group was more significant.Conclusion Continuous intravenous pumping of small doses of furosemide combined with oral enalapril has a sig-nificantly curative effect on the patients of chronic pulmonary heart disease with heart failure.关键词
