阻塞性睡眠呼吸暂停低通气综合征患者气道炎症及气管壁厚度变化
Variation of the Airway Inflammatory Level and the Bronchial Wall Thickness in Patients With Obstructive Sleep Apnea-hypopnea Syndrome

Abstract
Objective To observe the variation and correlative factors of airway inflammatory level and the bronchial wall thickness in patients with obstructive sleep apnea - hypopnea syndrome(OSAHS)by PAP treatment. Methods A total of 58 serious OSAHS patients(OSAHS group)admitted into the Department of Respiratory Medicine of the First Affiliated Hospital of Xinjiang Medical University from February 2012 to August 2014 were recruited. Another 51 healthy controls(control group) were also recruited from the Health Center during the same period. Exhaled nitric oxide concentration( eNO)and neutrophil count(NEUT),myeloperoxidase( MPO),IL-8 and TNF-α levels in bronchial induced sputum were measured. High -resolution CT scan was conducted,and the ratio of wall thickness to airway diameter in segmental bronchus( T/ D)and the percentage of airway wall area to total airway cross sectional area( WA% )were also observed. Analysis was made on the correlation between the above indicators and body mass index( BMI),apnea hypopnea index( AHI),the nocturnal lowest oxygen saturation(LSaO2 )and the time percentage of oxyhemoglobin saturation less than 90% (CT90). After 6 - month PAP therapy,the variation of the indicators above in OSAHS patients were assessed again. Results OSAHS group and control group were not significantly different in NEUT and T/ D(P ﹥ 0. 05 for all);the two groups were significantly different in MPO,eNO, IL-8,TNF-α and WA%(P ﹤ 0. 05 for all). In OSAHS group,eNO level was positively correlated with BMI,AHI and CT90 and was negatively correlated with LSaO2 level(P ﹤ 0. 05 for all);MPO was negatively correlated with LSaO2 and was positively correlated with CT90( P ﹤ 0. 05 for all);IL-8 level was negatively correlated with LSaO2 ( P ﹤ 0. 05);TNF-α level was positively correlated with BMI and was negatively correlated with LSaO2( P ﹤ 0. 05 for all). In OSAHS group,the levels of MPO,eNO,IL-8 and TNF-α after treatment were significantly different from those before treatment(P ﹤ 0. 05);NEUT,T/ D and WA% after treatment were not significantly different from those before treatment( P ﹥ 0. 05). Conclusion The airway inflammatory level in OSAHS patients is higher,and chronic airway inflammation can cause bronchial wall thickening. The levels of eNO,IL-8,TNF-α and MPO in OSAHS patients are related to the patients' BMI,sleep disorder and hypoxemia index. The PAP treatment could reduce the airway inflammatory level.关键词
