不同亚型遗忘型轻度认知障碍与轻度阿尔茨海默病弥散张量成像特点的比较研究
Comparison of Diffusion Tensor Imaging Characteristics Between Different Subtypes of Amnestic Mild Cognitive Impairment and Mild Alzheimer Disease

Abstract
Objective To investigate the changing characteristics of FA and ADC values in patients with two subtypes of amnestic mild cognitive impairment(aMCI)and mild Alzheimer disease(AD)by magnetic resonance diffusion tensor imaging (DTI),and to evaluate the application value of DTI in the diagnosis of aMCI and AD. Methods A total of 118 patients who accorded with inclusion criteria and were admitted into Tangshan Gongren Hospital from June 2012 to June 2015 were selected using MoCA,MMSE and AVMT. The patients were divided into three groups:AD group(n = 41),aMCI-s group(n = 37) and aMCI-m group( n = 40). Superconducting magnetic resonance instrument was employed to conduct axis TSE sequence continuous scanning. The original data were managed after scanning,and FA and ADC values of corpus callosum knee,corporis callosita splenium,parietal lobe,cooipital lobe,frontal lobe and gyrus cinguli were measured on B0 images and color tensor images at different levels;the measurement was controlled in region of interest(ROI)with 2 voxels. Data were all measured with blind method,and remeasurement and recalculation were made the next day,and average values were calculated. Results The three groups were not significantly in the FA value of corpus callosum knee,corpus callosum splenium,right parietal lobe,right occipital lobe and right frontal lobe(P ﹥ 0. 05). The three groups were significantly different in the FA value of left parietal lobe, right occipital lobe,left occipital lobe,left frontal lobe,right cingulum and left cingulum(P ﹤ 0. 05);the AD group was lower than the aMCI-s group in the FA value of left parietal lobe,left occipital lobe,left frontal lobe,right cingulum and left cingulum (P ﹤ 0. 05). The three groups were significantly different in ADC value among different brain regions(P ﹤ 0. 05);the aMCI-m group and the AD group were higher than aMCI-s group in ADC value of corpus callosum knee,corpus callosum splenium,right parietal lobe,right occipital lobe,left occipital lobe and right frontal lobe and left frontal lobe(P ﹤ 0. 05);the AD group were higher than aMCI-m group in ADC value of corpus callosum knee,corpus callosum splenium,right parietal lobe,left occipital lobe and right frontal lobe and left frontal lobe(P ﹤ 0. 05);AD group was higher than aMCI-s group and aMCI - m group in ADC value of left parietal lobe,right cingulum and left cingulum(P ﹤ 0. 05). Conclusion aMCI-s patients and aMCI-m patients are distinctly different in nerve fiber integrity,while there is no distinct difference in anisotropic diffusion,and ADC value is a potential imaging marker for aMCI subtyping.关键词
