腰硬联合麻醉应用于临产孕妇的分娩镇痛效果及其对妊娠结局的影响
Influences of Combined Spinal Epidural Anesthesia Applied to Pregnant Women on Labor Analgesia Effect and Pregnancy Outcome

Abstract
Objective:To discuss the influences of combined spinal epidural anesthesia(CSEA) applied to pregnant women on labor analgesia effect and pregnancy outcome.Method:174 primipara from January 2012 to September 2013 in our hospital were selected as the research group and randomly divided into the group A(n=60), group B(n=56),group C(n=58),the other 67 primipara without labor analgesia were selected as the control group.The control group was conventional obstetrics management,and the research group was given CSEA for labor analgesia,the group A,group B,group C were given CSEA when cervix opened up to 1,2,3 cm.The pain in each time period,progress of labor,usage of oxytocin,delivery mode and pregnancy outcome were compared among the four groups.Result:The VAS scores at each time period of group A,group B,group C were significantly lower than those of the control group,and group A and group B were significantly lower than that of the group C,the differences were statistically significant(P<0.05).The first stage of labor process in group A,group B and group C were significantly shorter than that in the control group,and group A,group B were significantly shorter than that in the group C,the differences were statistically significant(P<0.05).The usage of oxytocin in the group A,group B,group C were significantly higher than that in the control group,cesarean section rate were lower than that of the control group,the differences were statistically significant(P<0.05).There were significant differences of the maternal satisfaction scores among the four groups(P<0.05),group A>group B>group C>control group.There were no significant differences in the forceps delivery rate,2 h postpartum hemorrhage, neonatal 1 min and 5 min Apgar scores among the the four groups(P>0.05).Conclusion:CSEA applied to pregnant women can significantly shorten the first stage of labor,reduce the rate of cesarean section,and cervix opened up to 1,2 cm has better analgesic effects,and maternal satisfaction is high.关键词
