糖预处理联合胰岛素强化治疗对减轻术后胰岛素抵抗的价值
V alue of preoperative carbohydrate loading combined with intensive insulin therapy in reducing postoperative insulin resistance

Abstract
Objective To study the value of preoperative carbohydrate loading combined with intensive insulin therapy in reducing postoperative insulin resistance after open surgery. Methods 160 patients who were received open surgery in general surgery department were selected and they were randomly divided into the blank control group, preoperative carbohydrate loading group, intensive therapy group and combined treatment group, with 40 in each. Blank control group and the intensive therapy group were received preoperative fasting according to routine methods used at home and abroad. Preoperative carbohydrate loading group and combined treatment group were received fasting at 12 hours before operation and orally taken 50%GS 100ml. Intensive therapy group and combined treatment group were received Leuven intensive therapy. Venous blood samples were collected in fasting condition in the morning of the day when receiving operation, the same day after operation, and 1, 3, 7 days after the operation. Fasting blood glucose (FBG) and fasting insulin (FINS) were monitored. Insulin resistance index was calculated according to homeostasis model assessment (HOMA-2). Time for postoperative recovery time of bowel sounds, time for first flatus and complications were recorded. Results Insulin resistance indexes of the same day after operation, 1day and 3 days after the operation of the combined group were significantly lower than that of the other three groups. The difference had statistical significance (P<0.05). Compared with other three groups, flatus time of the combined group was advanced and incidence of complications was decreased, which had statistical significance (P < 0.05). Conclusion Compared with the single method treatment, preoperative carbohydrate loading combined with intensive insulin therapy in treatment of patients in the perioperative period can more effectively reduce perioperative insulin resistance. And it has certain curative effect for accelerating operative flatus and reducing postoperative complications.关键词
