基于达标理论的老年高血压糖尿病患者家庭访视效果观察
Effect of King’s Theory-based Home-visiting on Elderly Patients with Chronic Disease

Abstract
Objective To explore the effect of King’s theory-based home-visiting on elderly patients with chronic disease. Methods Totally 129 elderly patients with chronic disease in one community health service center were randomly divided into intervention group (n=65) and control group (n=64). Conventional home visiting was conducted in control group while King ’s theory-based home-visiting in intervention group for 3 months. Blood pressure, blood sugar and HbA1C in both groups were recorded and compared. Results In the end of intervention, blood pressure control in intervention group was better than that of`control group (P<0.01), and blood sugar and HbA1C control in intervention group were better than those in control group (P<0.05). Conclusion King’s theory-based home-visiting can significantly improve the control of blood pressure in elderly patients with hypertension, and the control of blood sugar and HbA1C in elderly patients with diabetics. It can develop disease management ability of old patients with chronic diseases.关键词
