上海市创伤急救病人院内诊治模式调查
Investigation on in-hospital first-aid of trauma patients in Shanghai
摘要
Abstract
Objective:To investigate the modes of trauma patients’ in-hospital first-aid in Shanghai and to promote the emergency system.Methods:Information about different modes of emergency cure for the trauma in hospitals were gathered through several methods including field visits,focus group discussion and expert interview. Administration organization system,setting of in-hospital trauma first-aid and its training standard in 14 hospitals were investigated and influence of different modes of trauma first-aid on mortality was compared.Results:Two trauma emergency modes were adopted by these medical institutions,including trauma center system(five institutions)and first diagnosis responsibility system of emergency department(or first-aid center general practitioners system)(nine institutions). The trauma center system was better than the other mode in trauma dedicated emergency room,emergency examination and allocation of operation room,grading score for patients and standard physician training. There were no significant differences between thetwo modes in constitutions of the physicians’ educational background,professional title,professional structure and professional background. Establishment of trauma database was imperfect in both two modes,which made it hard to provide objective appraisal of quality of trauma first-aid.Conclusion:Most of the hospitals in Shanghai still use first diagnosis responsibility system of emergency department(or first-aid center general practitioners system). However,trauma center system has more advantages in management mode. It is suggested that hospitals establish trauma center as soon as possible if conditions permit. Strengthening standardized training of trauma first-aid and establishing trauma database at municipal level are also urgent.关键词
