The relationship among red cell distribution width, neutrophil/lymphocyte ratio and non valvular atrial fibrillation in elderly patients with hypertension

Abstract
Objective To investigate the relationship between the red blood cell distribution width ( RDW) and neu-trophil/lymphocyte ratio ( NLR) and atrial fibrillation in elderly patients with essential hypertension.Methods From 2014 March to 2015 March, 200 cases of elderly hypertensive patients in the Department of Cardiology,Renmin Hospital of Wuhan University Hospital were enrolled, based on whether combined with non valvular atrial fibrillation, they were divided into AF group with 95 patients and control group with 105 patients.At the same time, the atrial fibrillation group was divided into par-oxysmal atrial fibrillation sub group (53 cases) and persistent/permanent atrial fibrillation sub group (42 cases).The basic clinical data of the patients and the results of laboratory examination, 24 h dynamic electrocardiogram and heart color ultra-sound examination were collected and analyzed.Results There was a statistically significant difference of NLR and RDW be-tween control group and AF group ( t =3.749,3.456, P <0.01).Compared with paroxysmal atrial fibrillation subgroup, persistent/permanent atrial fibrillation subgroup’s RDW and NLR increased significantly (all P <0.05).Multivariate logistic regression analysis showed that RDW is independent predictive factor in elderly hypertensive patients with atrial fibrillation (OR=0.344, 95%CI 0.152-0.777, P =0.01), and NLR is not independent predictive factor in elderly hypertensive pa-tients with atrial fibrillation (OR=0.573, 95%CI 0.271-1.209, P =0.144).ROC curve showed that RDW predict atrial fibrillation’s ROC area under the curve was 0.770 ( P =0.002, 95%CI 0.654-0.886), the best diagnostic cut off value was 13.0%and diagnostic sensitivity and specificity were 56.7%and 91.7%.Conclusion The RDW and NLR increased in elderly patients with hypertension and atrial fibrillation associated with non valvular disease, RDW is a prediction factor for hy-pertension and atrial fibrillation.关键词
