Abstract
Objective To investigate the willingness to accept appointed outpatient service among residents signing the contract of family doctor service and its influencing factors,to provide references for the promotion of appointed outpatient service and the improvement of the success rate of appointment in the future. Methods Among the 26 family doctor teams of Yuetan Community Health Service Center,we selected 8 teams using simple random sampling method. According to the lists of residents signing the contract of family doctor service provided by the teams and using simple random sampling method,we enrolled 80 residents from each team,and a total of 640 contract - signing residents were enrolled. In May 2015,questionnaire survey was conducted by trained and qualified medical staff on these residents. The questionnaire covered the basic information of contract -signing residents,the willingness to accept appointed outpatient service and the influencing factors for the willingness. We distributed 640 questionnaires,and 632 effective questionnaires were returned with an effective rate of 98. 8%. Results Among 632 contract - signing residents,538 residents knew that family doctors provide appointed outpatient service to contract - signing residents,with an awareness rate of 85. 1% . There were 213 residents willing to accept appointed outpatient service with an acceptance rate of 33. 7% . The reasons for residents accepting appointed outpatient service included relatively fixed doctors〔80. 8% (172 / 213)〕,doctors knowing about their diseases〔78. 9% (168 / 213)〕,and the avoidance of queuing up to save time〔60. 6% (129 / 213)〕. The reasons for residents not accepting appointed service included little difference between service with appointment and service without appointment〔74. 7% (313 / 419)〕,easiness to forget appointments〔54. 4% (228/ 419)〕,and time constraint for outpatient service〔48. 9% (205 / 419)〕. Logistic regression analysis showed that age,having chronic diseases or not,accepting the first diagnosis in community or not and being satisfactory with family doctor service or not were influence factors for the willingness to accept appointed outpatient service among residents signing contracts( P < 0. 05). Conclusion Contract-signing residents have low acceptance rate toward appointed outpatient service,and contract -signing residents aged 60 and above who have chronic diseases,accept first diagnosis in communities and are satisfied with family doctor service have higher acceptance rate toward appointed outpatient service. It is recommended that first diagnosis in medical settings at primary level,the improvement of medical insurance system,the quality of family doctor service,appointment process of outpatient service should be advanced to comprehensively implement appointed outpatient service.关键词
