Comparison of Clinical Efficacy and Cytotoxicity Among Dressings of Silver Ion,Rivanol and Iodophor

摘要

Abstract

Objective To investigate the efficacy,cost - effectiveness and cytotoxicity of commonly used antibacterial wound dressings of silver ion,rivanol and iodophor on infective wounds on the head and face. Methods A retrospective analysis was made on the data of 46 patients who got infection after debridement and suturing for skin laceration on the face caused by various reasons and received treatment in the Department of Emergency of Beijing Tongren Hospital Affiliated to Capital Medical University from July 2013 to April 2014,and all the patients were divided into silver ion group(n = 16),iodophor group(n= 15)and rivanol group( n = 15)in accordance with the antibactierial dressings used. The three groups were treated with corresponding wound dressings,and wound healing time and treatment expense were recorded. Chinese hamster lung(CHL)cells were seeded in a 96 - well plate,and iodophor(0. 000 5% - 0. 1% ),hydrogen peroxide(0. 002 5% - 0. 5% ),silver ion (0. 1% - 20% )and rivanol(0. 5% - 100% )were diluted into 8 different concentrations and were incubated with the cells for 6 hours. ELIASA was used to detect the absorbance value(OD value)of each well at 450 nm wave length,and the inhibition rate of cells proliferation was evaluated by OD(% ) = ( ODtest group / ODcontrol group ) × 100% . Results The three groups were not significantly different in healing time,7 - day healing rate and 14 - d healing rate( P > 0. 05). The three groups were significantly different in treatment expense(P < 0. 05);iodophor group and rivanol group were lower than silver ion group in&nbsp;treatment expense(P < 0. 05). After 6 - hour culture,the inhibition rate of cells proliferation was rivanol(41. 922% ) >silver ion(5. 528% ) > hydrogen peroxide(0. 058% ) > iodophor(0. 006% ). Conclusion Compared with iodophor and rivanol,silver ion show no difference in healing time or healing rate of the head and facial infection wounds,and the cytotoxicity on cell proliferation is medium,whereas the total treatment expense is higher. Patients could make their choice on wound dressings according to their immunity and economic ability.
