Abstract
ObjectiveTo explore the rational use of glucocorticoid in the treatment of patients who suffered acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and mechanical ventilation.Methods81 AECOPD patients with mechanical ventilation who were in the ICU of our hospital from July 2013 to April 2015 were divided into methylprednisolone (ME) group (42 cases) and prednisone (PR) group (39 cases). The two groups were treated with intravenous injection of methylprednisolone sodium succinate and oral prednisone acetate respectively. The curative effect and side effect of the two groups during treatment were estimated.Results The differences of the mechanical ventilation time, failure rates of noninvasive mechanical ventilation, ICU time, and ICU mortality between the two groups were not statistically significant (P>0.05). In the ME group, the hospitalization time was longer, and the hyperglycemia proportion was obviously higher than those of the PR group (P<0.05). The differences of the secondary infection rates, hypertension rates, and delirium rates between the two groups were not statistically significant (P>0.05).Conclusion For AECOPD patients with mechanical ventilation, excessive dose of glucocorticoid will raise the blood glucose and prolong their hospitalization time, and oral glucocorticoid is also suitable for them.关键词
