Abstract
Objective To investigate the relationship of lycated albumin ( GA) , adiponectin ( ADP) , retinol binding protein 4 (RBP4), homocysteine (Hcy) and gestational diabetes insulin resistance.Methods From July 2014 to July 2015, 80 cases of GDM patients ( GDM group) , 80 cases of normal pregnant women ( normal pregnancy group) and 80 cases of nor-mal non-pregnant women ( normal non-pregnant group) were collected in the Department of Gynecology, Baoding maternal and Child Health Hospital.The levels of GA, ADP, RBP4, Hcy of the 3 groups were compared to analyze the relationship of GA, ADP, RBP4, Hcy and gestational diabetes insulin resistance.Results The levels of GA, RBP4, Hcy of GDM group (18.36 ±0.98)%, (42.98 ±2.28) mg/L, (22.36 ±2.58) μg/L were significantly higher than those in normal pregnancy group (14.22 ±2.05)%, (34.22 ±3.48) mg/L, (1.25 ±2.98) μg/L and normal non-pregnant group (14.36 ±2.69)%, (30.02 ±2.96) mg/L, (11.02 ±2.79) μmg/L, while ADP (4.23 ±1.42) mg/L were lower than normal pregnancy group (9.23 ±2.58) mg/L and normal non-pregnant group (9.78 ±2.69) mg/L ( P <0.05), besides, the level of RBP4 of nor-mal pregnancy group was higher than normal non-pregnant group ( P <0.05).The levels of fasting blood glucose (FPG), fasting insulin ( FINS) , insulin resistance index ( HOMA-IR) of GDM group were higher than normal pregnancy group and normal non-pregnant group ( P <0.05), and the levels of FPG, FINS, HOMA-IR of normal pregnancy group were higher than normal non-pregnant group ( P <0.05), difference did not show statistically significant.The levels of GA, RBP4, Hcy were positively correlated with FPG, FINS, HOMA-IR ( P <0.05) , while the levels of FPG, FINS, HOMA-IR were nega-tively correlated with ADP ( P <0.05).Area under the ROC curve (AUC) of GA, ADP, RBP4, Hcy joint diagnosis of GDM was 0.902,which was greater than a single index, sensitivity(87.2%)and specificity(88.6%)were higher than a single indicator.Conclusion There is a close relativity of abnormalities of serum GA, ADP, RBP4, Hcy levels and insulin resist-ance in patients with GDM, by detecting the levels of serum GA, ADP, RBP4, Hcy can quickly and effectively diagnose GDM, and the joint detection of multiple indicators can improve the sensitivity and specificity of diagnoses .关键词
