Abstract
Objective To study the effect of double lumen bronchial tube on the success rate of resuscitation in the patients with respiratory and cardiac arrest caused by the injury.Methods 44 cases of prehospital respiratory and cardiac arrest caused by chest trauma (one-sided pneumothorax, pneumothorax) from February 2004 to February 2010 of our hospital were randomly selected as the control group. Resuscitation method: took the basic life support and gave the chest closed drainage, after the end of the tracheal intubation, and then performed cardiopulmonary resuscitation for 30 min, quickly transferred the patients with respiratory and heart recovery to the hospital for comprehensive treatment, and tracked the success rate of recovery. 44 cases of prehospital respiratory and cardiac arrest caused by chest trauma from March 2010 to March 2015 of our hospital were selected as the experiment group. Resuscitation method: took basic life support and double lumen bronchial intubation(10 s) at the same time, single-lung ventilation for injured pulmonary sequestration, and then performed cardiopulmonary resuscitation for 30 min, the patients with the recovery of respiratory and heart rate immediately underwent thoracic close drainage and double-lung ventilation, quickly transferred to the hospital for comprehensive treatment, and track the success rate of recovery.Results In control group, 20 patients were successfully resuscitated, 24 patients died, the recovery success rate was 45.45%(20/44), patients were given comprehensive treatment after successful cardiopulmonary resuscitation, and 5 patients were discharged, the hospital survival rate was 25%(5/20). In experimental group, 31 patients were successfully resuscitated, the recovery success rate was 70.45%(31/44), patients were given comprehensive treatment after successful cardiopulmonary resuscitation, and 10 patients were discharged, the hospital survival rate was 45.16%(14/31). The difference of success rate of cardiopulmonary resuscitation between two groups is statistically significant (χ2=5.64,P<0.05). The difference of discharge survival rate between two groups was statistically significant (χ2=3.48,P<0.05).Conclusions The basic life support and establishment of advanced airway by the double lumen bronchus for the patients with respiratory and cardiac arrest caused by chest trauma, can improve the success rate of cardiopulmonary resuscitation and discharge rate.关键词
