全髋与半髋关节置换治疗老年股骨颈骨折的疗效比较

Abstract
OBJECTIVE: To compare the clinical efficacy of total hip arthroplasty and hemiarthroplasty on femoral neck fractures in the elderly in order to choose better operation way for elderly patients.METHODS: The clinical data of 121 elderly patients with femoral neck fracture admitted in this hospital from January 2010 to January 2013 was retrospectively analyzed.Among these 121 pa-tients, 41 patients were treated by total hip arthroplasty and 80 patients were treated by artificial femoral head replacement.The ad-vantages and disadvantages of these two procedures were analyzed.RESULTS: All of these patients were successfully operated.Oper-ating time, amount of intraoperative blood loss and postoperative drainage in the patients in hemiarthroplasty group were better than those in total hip replacement group (P ﹤ 0.05).Their excellent rates of Harris scores were 87.8% and 71.2% respectively (P ﹤0.05).CONCLUSIONS: Both total hip replacement and hemiarthroplasty are effective procedures in the treatment of elderly patients with displaced femoral neck fractures.Those patients with long life expectancy and good tolerance choosing total hip arthroplasty are more favorable and they can acquire better postoperative joint function recovery.Hemiarthroplasty is more adequate for the patients with short life expectancy and bad health.关键词
