微创单侧固定与开放双侧固定治疗单间隙腰椎管狭窄症的疗效比效

Abstract
Objective To compare the therapeutic effect of the minimally invasive unilateral approach bilateral de-compression fixation and the open bilateral decompression fixation for single gap lumbar spinal stenosis. Methods The 33 patients of single gap lumbar spinal stenosis were randomly divided into two groups. In the minimally invasive unilateral group, 16 patients were treated by unilateral transforaminal bilateral decompression and unilateral fixation by using the channel. In the open bilateral group, 17 patients were treated by bilateral transforaminal bilateral decom-pression and bilateral fixation. The operative time, blood loss, hospital stay, intervertebral height index, ODI and VAS scores of the two groups were compared. Results The surgery time, blood loss, hospital stay of minimally inva-sive unilateral group were less than open bilateral group, these differences were statistically significant(P<0. 05). All patients were followed up from 12 months to 52 months. The ODI and VAS in the postoperative improved signifi-cantly when comparing with the preoperative ( P<0. 001 ) , but there was no significant difference between the two groups(P>0. 05). The disc height index of two groups were not statistically significant between 1 year and immedi-ately after surgery( P>0. 05 ) . Conclusions For single gap lumbar spinal stenosis, minimally invasive unilateral transforaminal bilateral decompression unilateral fixation has similar clinical and radiographic efficacy with open bilat-eral decompression fixation. Minimally invasive unilateral fixation has some advantages such as less trauma, shorter operative time,shorter hospital stay.关键词
