基于跨理论模型的动机性访谈对老年2型糖尿病患者血糖及自我管理行为的影响
Effect of Trans-theoretical Model -based Motivational Interviewing on Blood Glucose Level and Self -management Behavior of Elderly Patients With Type 2 Diabetes

Abstract
Objective To explore the effects of trans - theoretical model - based motivational interviewing on blood glucose level and self - management behavior of elderly patients with type 2 diabetes. Methods 162 elderly patients with type 2 diabetes who were admitted to Geriatrics Department of the Third Xiangya Hospital of Central South University from January 2014 to March 2015 were divided into experimental group(80 cases)and control group(82 cases)by the random number table method. Patients in experimental group received trans - theoretical model - based motivational interviewing on the basis of routine nursing and health education,while patients in control group received routine nursing and health education. Before intervention and 6 months after intervention,levels of fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c )and 2 - hour post -meal blood glucose(2 hPBG),type 2 diabetes self - management behavior scale score( the scale included 6 contents:diet control,physical exercise,take the medicine according to the instruction,blood sugar monitoring,foot care,abnormal blood sugar processing)were compared between two groups. Results There was no significant difference in levels of FBG,2 hPBG and HbA1c before intervention and level of 2 hPBG after intrvention between two groups(P > 0. 05). After intervention,levels of FBG and HbA1c of patients in experimental group were significantly lower than those of patients in control group,respectively(P< 0. 05). Before intervention,there was no significant difference in scores of diet control,physical exercise,take the medicine according to the instruction,blood sugar monitoring,foot care and abnormal blood sugar processing between two groups(P >0. 05). After intervention,scores of diet control,physical exercise,take the medicine according to the instruction,blood sugar monitoring,foot care and abnormal blood sugar processing of patients in experimental group were significantly higher those of patients in control group, respectively ( P < 0. 05 ) . Conclusion The trans - theoretical model - based motivational interviewing can help control blood glucose level and improve self - management behavior among elderly patients with type 2 diabetes.关键词
