椎体成形术与椎体后凸成形术治疗骨质疏松性椎体压缩性骨折发生骨水泥渗漏的相关临床评价
Clinical Evaluation Related Bone Cement Leakage after Vertebroplasty and Kyphoplasty in Treatment of Pa-tients with Osteoporotic Vertebral Compression Fractures

Abstract
Objective To preliminarily investigate the incidence rate and related factors of bone cement leakage after vertebroplasty and kyphoplasty in treatment of patients with osteoporotic vertebral compression fractures. Methods Clinical data of 178 thoracolumbar osteoporosis vertebral compressional fracture patients undergoing vertebroplasty or ky-phoplasty during March 2012 and July 2014 was retrospectively analyzed. There were 71 males and 107 females with a mean age of 79. 6 years(69-89 years). The patients were treated with vertebroplasty(n = 81)and kyphoplasty(n = 97) respectively according to the patient’s choice. Incidence rate and distribution of bone cement leakage were analyzed with postoperative iconography examination in the two groups. Results The injected volume of bone cement of all patients was 1. 5-7. 0 ml. The incidence rates of bone cement leakage were 34. 57% ( 28 / 81 )in vertebroplasty group and 12. 37%(12 / 97)in kyphoplasty group respectively,and the differences were statistically significant( P ﹤ 0. 05). In vertebroplasty group,there were 8 patients with leakage in paravertebral soft tissues,13 patients with vertebral anterior edge leakage,2 patients with paraspinal veins leakage,3 patients with leakage in pedicle of vertebral arch and 2 patients with leakage in intervertebral space;in kyphoplasty group,there were 3 patients with leakage in paravertebral soft tis-sues,7 patients with vertebral anterior edge leakage and 2 patients with leakage in pedicle of vertebral arch. No bone ce-ment leakage in rearward and inner spinal canal was found in the two groups. Conclusion Vertebroplasty may achieve lower percolation rate of bone cement and better safety in treatment of patients with osteoporotic vertebral compression fractures than those by kyphoplasty.关键词
