非糖尿病胃癌手术患者营养支持期间应激性高血糖现状及影响因素分析
Status and Influence Factors of Stress Hyperglycemia in Perioperative Nondiabetic Gastric Cancer Patients during Nutritional Support

Abstract
Objective To investigate the current status and its influence factors of stress hyperglycemia in perioperative nondiabetic gastric cancer patients during nutritional support. Methods Totally 216 patients were investigated with a self-designed demographic data questionnaire, Nutrition Risk Screening 2002 (NRS 2002), Zung Self-rating Anxiety Scale (SAS) and Zung Self-rating Depressive Scale (SDS). Results The incidence of stress hyperglycemia was 32.9% and most patients’ blood glucose returned to normal at the 3rd day after surgery. Logistic regression analysis indicated that operation duration ( P=0.044), nutritional risk (P<0.001), depression (P=0.002), anxiety (P=0.003), age (P=0.008), nutritional support way (P=0.009) and operation method (P=0.038) were influence factors of stress hyperglycemia. Conclusion In order to stabilize patients’ blood sugar levels, great attention should be paid to the influence factors of stress hyperglycemia in perioperative nondiabetic gastric cancer patients during nutritional support. Clinical staff should evaluate patients ’ nutritional status in time, ease patients ’ bad psychological emotions, choose suitable operation type, shorten operation time and particularly focus on the aged. Enteral nutrition should be given to patients as early as possible after surgery and patients’ blood glucose should be monitored for at least 3 days.关键词
