Abstract
Objective To explore the application effect of" hospital - community" joint pressure sore care group in stroke patients with pressure sores on continuity of care after discharged. Methods From January 2011 to December 2013,we enrolled one hundred stroke patients with pressure sores who were hospitalized in the neurological department of a third-level hospital in Beijing and will be discharged from the hospital. During hospitalization, two groups were both administrated with routine pressure sore nursing. Before and after discharged,the control group was treated with conventional education and follow-up and the observation group was treated with continuity of care served by " hospital - community" joint pressure sore group. Before discharged,we recorded the two groups of general information,ability of daily living( ADL)table scores and Braden scale scores. Before discharged and discharged after one month,three-month,six-month,we recorded two groups of pressure ulcer scale for healing scores and pressure sore stage. Results The two groups were not significantly different in gender, average age,widowed,education level,categories of chronic diseases,average ADL scores and average Braden scale scores (P﹥0. 05). The observation group was lower than the control group in the score of pressure ulcer healing scale after discharge (P﹤0. 05). The two groups were significantly different in the score of pressure ulcer healing scale at different time points(P﹤0. 05). There was interaction between time points and groups(P﹤0. 05). The two groups were not significantly different in pressure sore stage before discharged(P﹥0. 05). The pressure sore staging of the observation group was better than that of the control group 1,3 and 6 months after discharge(P﹤0. 05). Conclusion The application effect of" hospital - community"joint pressure sore care group in stroke patients with pressure sores on continuity of care after discharged is good. It could help to heal the wounds of patients with pressure sores.关键词
