Research Progress of Chronic Disease Management
摘要
Abstract
With the rapid development of economy and society,the prevalence and mortality rate of chronic disease are increasing year by year. Based on some latest literatures about chronic disease management at home and abroad are consulted, the concept,the meaning and the object of chronic disease management,and the advances of chronic disease management at home and abroad are reviewed. Meanwhile,the chronic disease management model in our country in the future is prospected. With the support of community,continuous and comprehensive chronic disease management performed by a combination of department of general practice in class 1 & grade 3 general hospitals,special hospitals and primary hospitals,can improve the level of standardized management of community general practitioners in the field of chronic disease management,and can also improve the awareness and the control rate of chronic disease.关键词
