辅助性 T 淋巴细胞17/调节性 T 淋巴细胞平衡在乙型肝炎病毒感染相关肝脏疾病进展中的作用研究
Influence of the Balance of Th17/Treg in the Progress of Hepatitis B Virus Related Liver Disease

Abstract
Objective To explore the role of the balance of Th17 / Treg in the progress of hepatitis B virus(HBV) related liver disease. Methods From January 2011 to June 2015,we enrolled 111 patients with HBV infection who accorded with inclusion and exclusion criteria from the First Affiliated Hospital of Kunming Medical University. Base on the diagnostic standards of HBV in Practical Infectious Diseases ,all the patients were divided into four groups:group A(23 patients with asymptomatic chronic HBV carriers),group B(42 patients with mild - moderate chronic hepatitis B patients),group C(18 patients with severe chronic hepatitis B)and group D(28 patients with hepatitis and hepatic cirrhosis),and we also enrolled 11 healthy people who received treatment in the physical examination center in the hospital in the same period as control group. Realtime fluorescent quantitative system was used to determine HBV - DNA load,and flow cytometry was employed to detect the expression rates of peripheral Th17 and Treg and Th17 / Treg. Results Group A,group B,group C,group D were not significantly different in the HBV - DNA load(P > 0. 05). The 5 groups were not significantly different in the expression rate of Treg(P >0. 05);group A was lower than control group in the expression rate of Th17 and Th17 / Treg,and group B was lower than control group in Th17 / Treg(P < 0. 05);group B,group C and group D were higher than group A in the expression rate of Th17 and Th17 / Treg(P < 0. 05);group C was higher than group B in the expression rate of Th17 and Th17 / Treg(P < 0. 05). For the all patients with HBV infection,the expression rate of Th17 was 2. 7% (3. 2% ),the expression rate of Treg was 1. 0% (1. 2% ), and HBV - DNA load was 66. 0 × 104 (1 206. 7 × 104 )copies / ml. The expression rate of Th17 was positively correlated with the expression rate of Treg(rs = 0. 201,P = 0. 026);HBV - DNA load had no linear correlation with the expression rate of Th17(rs = 0. 137,P = 0. 257),the expression rate of Treg( rs = - 0. 019,P = 0. 878),and Th17 / Treg( rs = 0. 120,P= 0. 324). Conclusion The imbalance of Th17 / Treg maybe a common phenomenon in patients with chronic HBV infection, and the ratio of Th17 / Treg can more accurately reflect the progress of HBV infection - related liver disease than the expression rate of Th17 or Treg alone.关键词
