抗病毒治疗对慢性乙型肝炎患者外周血辅助性 T 淋巴细胞17及调节性 T 淋巴细胞相关细胞因子的影响研究
Impact of Antiviral Therapy on the Related Cytokines of Th17 and Treg in the Peripheral Blood of Patients With Chronic Hepatitis B

Abstract
Background The role of Th17 and Treg in hepatitis B virus( HBV)infection is being recognized gradually. However,the impact of antiviral therapy on the related cytokines of Th17 and Treg is still unclear. Objective To explore the impact of antiviral therapy on the related cytokines of Treg and Th17 in the peripheral blood of patients with chronic hepatitis B. Methods From October 2011 to August 2015,we enrolled 86 patients with chronic hepatitis B(observation group) who received treatment in the Department of Infectious Disease of the First Affiliated Hospital of Kunming Medical University and enrolled 20 healthy people(control group)in the same hospital. Virological responses of the patients were observed,including complete virological responses(complete response group)and incomplete virological responses(incomplete response group). ELISA method was used to detect the related cytokines( IL-17、TGF-β、IL-10、IL-23 and IL-21)of Th17 and Treg in the peripheral blood of the subjects. Results Control group and observation group were significantly different in the levels of IL-17, TGF-β,IL-10,IL-23 and IL-21(P < 0. 05). The levels of IL-17,TGF-β,IL-10,IL-23 and IL-21 of observation group after treatment were significantly different from those before treatment( P < 0. 05). Among the 86 patients,79(91. 9% )patients had complete virological responses and 7(8. 1% ) patients had incomplete virological responses. Complete response group and incomplete response group were not significantly different in the levels of IL-17,TGF-β,IL-10,IL-23 and IL-21 before treatment(P > 0. 05). Complete response group and incomplete response group were significantly different in the level of IL-17 after treatment(P < 0. 05). Before treatment,the level of IL-17 was positively correlated with the levels of IL-10 and IL-23(P< 0. 05);the level of TGF-β was negatively correlated with the level of IL-10,and was positively correlated with IL-21(P <0. 05);the level of IL-10 was positively correlated with IL-23(P < 0. 05). The decrease of IL-17 level and the decrease of IL-10 and IL-23 levels were positively correlated(P < 0. 05);the decrease of TGF-β level was negatively correlated with the decrease of IL-10 level,and was positively correlated with the decrease of IL-21 level(P < 0. 05);the decrease of IL-10 level was positively correlated with the decrease of IL-23 level(P < 0. 05). Conclusion Antiviral therapy can lower the levels of the related cytokines of Th17 and Treg and control the progress of liver disease,TGF-β may play a bridging role in a series of cytokines regulation.关键词
