脉搏指示连续心输出量技术目标导向液体复苏治疗在烧伤早期中的应用研究
Application of PiCCO Goal -directed Therapy in Early Fluid Resuscitation of Burn

Abstract
Objective To assess the difference of pulse indicator continuous cardiac output(PiCCO)technology and the infusion by traditional formula in early fluid resuscitation of burn. Methods From January 2010 to January 2014,we enrolled 13 burn patients who accorded with the inclusion and exclusion criteria from the Department of Critical Care Medicine of the First Affiliated Hospital of Guangxi Medical University. Using random number table method,we divided the patients into two groups:research group(n = 7)and control group( n = 6). Fluid resuscitation was conducted under the guidance of PiCCO technology in research group and according to traditional infusion formula in control group,and was conducted with the guidance of traditional vital signs monitoring,urine volume and central venous pressure( CVP). General data,daily average fluid infusion volume,daily average urine volume,daily average heart rate,daily average mean arterial pressure( MAP),daily average CVP and the dosage of vasoactive drugs,intrathoracic blood volume index(ITBI),cardiac index(CI)and CVP on day 1 to day 3,serum creatinine level on day 3,blood urea nitrogen level on day 3,the length of ICU stay,case fatality rate. Results Research group was higher than control group in daily average fluid infusion volume and daily average urine volume (P < 0. 05);research group was lower than control group in average daily heart rate,daily average MAP and the dosage of vasoactive drugs(P < 0. 05);the two groups were not significantly different in daily average CVP(P > 0. 05). Research group had higher ITBI and CI on day 2 and 3 than those on day 1,and had higher ITBI and CI on day 3 than those on day 2(P <0. 05). The CVP of patients in research group was not significantly different on day 1 to day 3(P > 0. 05). The two groups were not significantly different in serum creatinine level and blood urea nitrogen level on day 3(P > 0. 05);research group was lower than control group in the length of ICU stay(P < 0. 05). The case fatality rate was 3 / 7 in research group and 3 / 6 in control group,and the two groups were not significantly different in the case fatality rate(P = 1. 000). Conclusion The application of PiCCO technology in the guidance of early fluid resuscitation of burn helps with the accurate determination of liquid load to reach resuscitation goal as soon as possible.关键词
