高频彩超与磁共振成像对类风湿性腕关节早期病变的诊断价值分析
Analysis of diagnostic value of high frequency color Doppler ultrasound and MRI in the diagnosis of early rheumatoid arthritis

Abstract
Objective:To investigate the diagnostic value of high frequency color Doppler ultrasonography and MRI in the diagnosis of early rheumatoid arthritis.Methods: 152 cases of rheumatoid arthritis (RA) patients in our hospital from March 2015 to November 2015 were selected as the research objects. At the same time, 50 cases of healthy persons were selected as the control group. Then high frequency ultrasound and MRI were conducted on 304 wrists of the 152 patients and 100 wrists of the 50 persons in the control group. The detection ability of imaging methods for the early lesions of RA wrist joint was compared between the two groups.Results:The detection rates of high frequency ultrasound on synovitis of wrist joint, joint effusion, tenosynovitis, and bone erosion of RA patients were respectively 100.00%, 67.76%, 61.51% and 39.14%, those of MRI were respectively 100.00%, 69.41%, 65.79% and 56.58%. Thus there were no obvious differences in detection rates of high frequency ultrasound and MRI carpal joint effusion, synovitis, tenosynovitis in RA patients(x2=0,x2=0.191,x2=1.201;P>0.05). But as for bone erosion, the detection rate of high frequency ultrasound was significantly lower than that of MRI (x2=18.514;P<0.01). And the average synovial thickness of RA patients was significantly higher than that in healthy control group(t=8.59,P<0.05).Conclusion: The detection rate of high frequency color Doppler ultrasound in the early lesions of RA wrist joint has high consistency with MRI. While as the preferred diagnostic tool of RA wrist joint early lesions, ultrasonographic features have no typical signs on some patients who can be further given MRI examination.关键词
