Abstract
Objective:To investigate the relationship between the level of microalbuminuria (MAU) and cardiac functional classification of chronic heart failure (CHF).Method:98 patients with chronic heart failure (CHF) were selected in our hospital from June 2014 to June 2015,they were divided into cardiac compensation group of 47 cases and cardiac deeompensation group of 51 cases according to NYHA standards.A total of 52 cases with normal physical examination were selected as control group at the same period.Glomerular filtrationrate(GFR), urinary albumin/creatinine ratio(UACR),MAU,brain natriuretic peptide(BNP),left ventricular ejection fraction(LVEF),left ventricular end-diastolic dimension(LEVDd) and left ventricular fractional shortening (LVFS) were measured and compared among the three groups.The correlation of cardiac function classification and UACR were analyzed.Result:The level of SCr and UMA in cardiac compensation and decompensation group were significantly higher than those of control group,these of decompensation group were significantly higher than compensation group,the differences were statistically significant(P<0.05).However,the level of GFR in cardiac compensation and decompensation group were lower than that of control group,that of decompensation group were significantly higher than compensation group,the differences were statistically significant(P<0.05). The level of BNP in decompensation group was highest in three groups,and compensation groups was second,the differences were statistically significant(P<0.05).LEVDd of cardiac compensation and decompensation group were significantly higher than control group,LVEF and LVFS of cardiac compensation and decompensation group were lower than control group,the differences were statistically significant(P<0.05).There were no significant differences between decompensation group and compensation group about LVEF,LVFS and LEVDd (P>0.05). There were significant differences in UACR levels among different NYHA grades(F=56.79,P<0.05),UACR level was positively correlated with the grade of cardiac function (r=0.61,P<0.001),while chronic cardiac insufficiency different NYHA classification with GFR and SCr had no correlation(P>0.05).Conclusion:The level of UACR can be used to evaluate the severity of chronic heart failure.关键词
