实时荧光定量聚合酶链反应检测尿沉渣中miR-146a的表达及在类风湿关节炎中的临床意义
Expression of miR-146a Detected by Real-Time Quantitative Polymerase Chain Reaction Assay in the Urinary Sediment of Patients with Rheumatoid Arthritis and Its Clinical Significance

Abstract
Objective To explore the expression of miR-146a detected by real-time PCR in the urinary sediment of patients with rheumatoid arthritis ( RA ) , with an attempt to explore its clinical significance in RA. Methods The morning midstream urine of 80 cases of RA patients ( 46 cases active, 34 cases inactive ) and 40 cases of healthy controls were collected and the RNA was extracted from the urine sediment. The expression level of miR-146a was detected by real-time quantitative polymerase chain reaction ( real time PCR ) and the correlation with disease activity in RA and RA clinical manifestations were analyzed. Results The expression level of miR-146a in RA patients was ( 0. 70 ± 0. 04 ) , which was significantly higher than ( 0. 17 ± 0. 01 ) in the healthy controls ( P < 0. 05 ) . The expression levels of miR-146a in active RA patients was ( 0. 91 ± 0. 05 ) , which was higher than ( 0. 42 ± 0. 04 ) in inactive RA pa-tients ( P < 0. 05 ) . The expression levels of miR-146a in inactive RA patients was higher than in the healthy controls [ ( 1. 17 ± 0. 01 ) vs ( 0. 63 ± 0. 04 ) , P < 0. 05 ] . The expression levels of miR-146a in RA patients with extraarticular manifestations was ( 1. 13 ± 0. 12 ) , which was higher than in RA patients without extraarticular manifestations ( 0. 59 ± 0. 04 ) ( P < 0. 05 ) . The expression level of urinary miR-146a was positively correlated with disease activity score in 28 joints(DAS28) ( r=0. 675, P < 0. 05), ESR( r=0. 657, P < 0. 05) and CRP( r=0. 522, P < 0. 05 ) , but not correlated with disease duattion ( r=0. 052, P= -0. 650 ) , RF ( r=0. 049, P=0. 664 ) and anti-CCP antibodies ( r=0. 089, P= -0. 443 ) . Conclusion The expression level of urinary miR-146a was significantly higher in RA patients and correlated with some clinical features of RA, indicating that miR-146a may play a role in the pathogenesis of RA.关键词
