低血糖及严重低血糖对老年2型糖尿病患者心血管事件的影响

Abstract
Objective:To explore the relationship between hypoglycemia,severe hypoglycemia and cardiovascular event by hazard ratio calculation, investigate their role in the progress of cardiovascular disease.Method:A total of 11 153 old patients with type 2 diabetes were selected,Cox proportional-hazards models with adjustment for covariates was used to evaluate the relationship with cardiovascular event,Charlson index sum used ANCOVA models with covariates to evaluate the relationship with comprehensive health status.Result:During a mean follow-up period of 5.88 years,4874 patients(43.7%) had at least one hypoglycemic episode,432 patients(3.9%) had at least one severe hypoglycemic episode;hypoglycemia was associated with a significant decrease in the adjusted risks of cardiovascular events(HR:0.837,95%CI:0.763,0.918,P<0.001) and significant yearly increase of Charlson index sum,severe hypoglycemia was associated with a significant increase in the adjusted risks of cardiovascular events(HR:1.718,95%CI:1.197,2.468,P<0.05) and significant yearly increase of Charlson index sum for those without a history of CV disease.Conclusion:Hypoglycemia is a comprehensive index of disease progression instead of cardiovascular risk marker;severe hypoglycemia was strongly associated with increased risks of cardiovascular event for those without a history of CV disease.关键词
