腹腔镜手术与开腹手术治疗Ⅰ期子宫内膜癌的近期及远期临床疗效观察
Long-term and Short-term ClinicalEffects of Open Operation and Laparoscopic Op eration on Stage I Endometrial Cancer

Abstract
Objective:To compare the long-term and short-term clinical effects of open operation and laparoscopic operation on stage I endometrial cancer, and to provide theoretical foundations for choice of op-eration way.Method:Data of forty patients undergoing traditional operation ( open operation group) and forty-three patients undergoing laparoscopic operation ( laparoscopic operation group) were collected, and the op-erative times, intra-operative blood losses, numbers of intra-operatively cleared lymph nodes, times to pas-sage of gas by anus, drainage volumes of abdominopelvic cavity, times to out-of-bed activity, postoperative retention times of urinary catheter, postoperative lengths of stay, complications,urination function indexes , and recurrence and metastasis conditions were compared.Rseult:The patients of the two groups had similar operative times and numbers of intra-operatively cleared lymph nodes, and the differences were statistically insignificant ( P>0.05) .The patients of the laparoscopic operation group had better intra -operative blood loss, time to passage of gas by anus, drainage volume of abdominopelvic cavity , time to out-of-bed activity, postoperative retention time of urinary catheter, and postoperative length of stay than the open operation group, and the differences were statistically significant ( P<0.05) .The patients of the laparoscopic operation group had shorter times to sensation of filling of bladder, satisfied urinary output, and residual urine volume was less than 100 mL than the open operation group, and the differences were statistically significant ( P<0. 05) .The incidence of complications in the patients of the laparoscopic operation group was 9.30%, which was lower than that of the open operation group ( 30.00%) , and the difference was statistically significant ( P<0.05) .Of the laparoscopic operation group, 4 patients ( 9.30%) had local recurrence and 3 patients ( 6. 98%) had metastasis, while of the open operation group, 3 patients ( 7.50%) had local recurrence and 2 patients (5.00%) had metastasis, and the differences were statistically insignificant (P>0.05).Conclusion:Laparoscopic operation in treatment of stage I endometrial cancer can obtain same long-term effects as tradi-tional open operation, and had smaller operation wound and rapid postoperative recovery, is beneficial to protecting bladder function, and consequently is worthy of clinical application.关键词
