妇科腹腔镜手术中气腹与非气腹对呼吸、循环功能及眼内压影响的比较
Comparison of circulatory, respiratory and intraocular pressure changes in gynecological laparoscopic surgery：be-tween carbon dioxide pneumoperitoneum and gasless laparoscopy

Abstract
Objective To observe the gynecological laparoscopic surgery in pneumoperitoneum and non pneumoper -itoneum on under general anesthesia hysterectomy patients with respiratory and circulatory function and intraocular pressure (IOP).Methods From January 2013 to June 2015, 60 cases of ASA grade I to grade II in the Department of gynecology and obstetrics of the Third Hospital of Shijiazhuang were selected as the research objects , they were randomly divided into 2 groups: pneumoperitoneum pressure group (P group, n =30), non pneumoperitoneum pressure group (NP group, n =30). Monitoring of patients in the two groups before anesthesia (T0 ) and the general anesthesia after 5 min (T1 ), pneumoperitone-um or dangling belly after 10 min (T2 ), 30 min (T3 ), the end of pneumoperitoneum or put down the abdominal wall after 20 min ( T4 ) breath at the end of the carbon dioxide partial pressure ( PET CO2 ), arterial carbon dioxide partial pressure (PaCO2 ), airway pressure (PAW), systolic blood pressure (SBP) and diastolic blood pressure (DBP), mean arterial pres-sure (map), heart rate (HR), intraocular pressure (IOP), and compared these indices .Comparison with T0 , P group T2 , T3's PET CO2 , PaCO2 , Paw, MAP, HR and IOP were significantly increased , the difference was statistically significant ( P <0.05), only IOP significantly increased in NP group , other indicators did not change significantly ( P >0.05).Compared with the NP group, P group at T0 , T1 , T4 , the monitoring index's difference were not statistical significance ( P >0.05), at T2 , T3 , PET CO, PaCO2 , paw, map, HR and IOP were significantly higher, the differences were statistically significant (P <0.05).Results These data were no differences between the two groups at T 0 and T1 .MAP、HR、PET CO2、Paw、PaCO2 and IOP of the P group were higher significantly at T 2 and T3 ( P <0.01).MAP、HR、PET CO2、Paw、PaCO2 and IOP of the P and NP groups were higher significantly at T 2 and T3 ( P <0.01).Conclusion CO2 pneumoperitoneum laparoscopic uterine re -section surgery has effect on respiratory and circulatory function and intraocular pressure , and gasless laparoscopic surgery , al-though have certain influence on intraocular pressure (IOP), but a gasless laparoscopic surgery significantly reduce and avoid CO2 pneumoperitoneum on respiratory and circulatory function.关键词
