医学生沟通能力现状及其影响因素调查研究
Investigation on the Current Situation and Influencing Factors of Medical Students′Communication Ability

Abstract
Objective To study the current situation of medical students′ communication ability,and to investigate the influencing factors. Methods From October 2013 to January 2014,multi stage stratified random sampling method was used to conduct questionnaire survey among 1 600 medical students from five grades of three medical colleges of Shandong Province, the questionnaire included general information questionnaire,personality extraversion questionnaire,doctor - patient cognition, communication knowledge questionnaire and interpersonal communication ability evaluation scale. Medical students′ doctor -patient relationship cognition and communication knowledge,interpersonal communication ability,and characteristics of introvert and extrovert personality were investigated and evaluated. According to the medical students′ communication ability score,those whose score reached 80% of the total scores and above were included into group A;those whose score < 80% of the total scores were included into group B. Influencing factors for medical students′ communication ability were analyzed. Results A total of 1 600 questionnaires were distributed,1 460 were recovered,1 381 were effective,the effective recovery rate was 86. 3%. Among 1 381 medical students,455 cases ( 32. 9% ) showed extrovert personality,598 cases ( 43. 3% ) showed intermediate personality,328 cases( 23. 8% ) showed introvert personality. 612 cases ( 44. 3% ) considered that the doctor - patient relationship will become better,551 cases(39. 9% )considered that the future of doctor - patient relationship was uncertain, and 218 cases(15. 8% ) considered that the doctor - patient relationship will become worse. The average score for medical students′ communication ability was(115. 9 ± 20. 1),254 cases were included into group A,the average score was(142. 7 ± 10. 0);group B included 1 127 cases,the average score was( 109. 7 ± 16. 5 ). There was no significant difference in communication ability among different medical student groups by gender and grade(P > 0. 05). Different origin of students,the only child,personality types,student cadres,family atmosphere,domestic economy status,doctor - patient communication course and the attitude of doctor - patient relationship had significant influence on the communication ability of medical students (P < 0. 05 ). Multiple Logistic regression analysis showed that the personality type,student cadres,family atmosphere, domestic economy status,doctor - patient communication course and the attitude of doctor - patient relationship had significant influence on the communication ability of medical students( P < 0. 05). Importance quadrantal diagram results showed that family atmosphere and family economic status were in zone Ⅱ,degree of importance was high,but the score was low;personality type and whether participating in doctor - patient communication course were in zone Ⅳ,and both degree of importance and score were high,which should should be maintained. Conclusion The medical students have a low level of communication ability. Major influencing factors for their communication ability include personality type,student cadres,family atmosphere,domestic economy status,doctor - patient communication course and the attitude of doctor - patient relationship. The communication ability of medical students can be improved measures such as self confidence exercise,increasing the opportunity of communicating with others,building harmonious family atmosphere and setting up standard courses for doctor - patient communication.关键词
