Abstract
Objective To predict the risk assessment of the short term cerebral infarction (CI) in internal carotid artery system TIA patients by modified ABCD2 scoring (ABCD2+L) method combined with fibrinogen (Fib), homocysteine (Hcy) and uric acid (UA).Methods The clinical data of 177 patients who suffered from internal carotid artery system TIA from January 2012 to December 2013 in neurology department of First Affiliated Hospital of Harbin Medical University were selected. Risk factor, Fib, Hcy, and UA value were recorded by ABCD2+L scoring method, all the patients were admitted to hospital in less than 24 hours, who were divided into low-risk group(score 0-3), moderate-risk group(score 4-5) and high- risk group(score 6-8) by ABCD2+L score. The CI incidence rate within 7d of the three groups were observed, the efficiency of ABCD2+L score was analyzed, compared with relevant laboratory indexes, the relationship for secondary CI risk was analyzed.Results In 177 internal carotid artery system TIA patients, there were 38 cases with CI in 7d, and 139 cases without CI, the CI incidence rate was 21.47%. There was statistically significant difference in CI incidence rate among the three groups (P<0.05). Multivariate Logistic regression analysis showed that age, hypertension, and homocysteine were independent risk factors for CI in internal carotid artery system TIA. The LDL-C and UA contents of the high-risk group, moderate-risk group, and low-risk group were compared, the difference of LDL-C and UA contents were statistically significant among the three groups (P<0.05). The difference of Fib and Hcy were no statistically significant among the three groups (P>0.05).Conclusion(1) Age, hypertension, and high Hcy were independent risk factors for short term CI in internal carotid artery system TIA patients. (2) The higher ABCD2+L score of internal carotid artery system TIA patients, the higher short term CI. (3) The higher LDL-C and UA value of internal carotid artery system TIA patients, the greater short term CI risk.关键词
