雷珠单抗联合玻璃体切割术治疗增生性糖尿病视网膜病变合并Ⅰ、Ⅱ期青光眼临床观察
Clinical Observation on Ranibizumab Combined with Vitrectomy in Treatment of Patients with Proliferative Dia-betic Retinopathy Accompanied by Stage Ⅰ and Ⅱ Glaucoma

摘要
Abstract
Objective To investigate clinical effect of Ranibizumab combined with vitrectomy (PPV) in treat-ment of patients with proliferative diabetic retinopathy ( PDR ) accompanied by stage I and II neovascular glaucoma (NVG). Methods Clinical data of 60 patients (60 eyes) diagnosed as having PDR accompanied by stage I and II NVG during June 2011 and June 2014 was retrospectively analyzed. The patients were divided into observation group (n=34, 34 eyes) and control group (n=26, 26 eyes) according to whether or not preoperative Ranibizumab injection was per-formed, and all the patients underwent 23G PPV. Observation group was injected with Ranibizumab before surgery, while control group was not injected with Ranibizumab before surgery. Surgical time, average time of neovessels bleeding, time of using electric coagulation and incidence rates of complications during operation and 6 months after operation in two groups were observed. The best corrected visual acuity ( BCVA ) , aqueous humor vascular endothelial growth factor ( VEGF) , pigment epithelium derived factor ( PEDF) before operation and 1 month after operation, and macula central fovea retinal thickness ( CRT) 1 month after operation were detected in two groups. Results Compared with those in control group, in observation group, values of surgical time, average time of neovessels bleeding and time of using electric coagulation were significantly lower ( P<0. 05 ); values of aqueous humor VEGF and PEDF before and after operation were significantly lower, while postoperative BCVA value was higher (P<0. 01);incidence rates of intraoperative iatro-genic retinal holes, intraoperative massive hemorrhage, postoperative transient ocular hypertension and vitreous hemor-rhage were significantly lower (P<0. 05). Conclusion Ranibizumab combined with PPV in treatment of patients with PDR accompanied by NVG can improve aqueous humor VGEF and PEDF, increase BCVA level and reduce incidence rates of intraoperative and postoperative incidence rates of complications.关键词
