Abstract
Objective To investigate the effect of surgical treatment on temporal lobe space-occupying lesion in patients with epilepsy ,so as to analyze and use the multiple localization techniques of epileptogenic focuses that may play a role in surgi‐cal efficacy.Methods We recruited 31 cases with temporal lobe space-occupying lesion and epilepsy ,whose preoperative EEG and postoperative follow-up clinical data were retrospectively analyzed.Of selected 31 cases ,7 cases presented cystic lesions ,6 cases presented cavernous hemangioma ,16 cases suffered from glioma ,and the rest underwent other lesions.Additionally , MRI ,video EEG ,MEG line were applied to locate epileptogenic focuses.Furthermore ,after using electrocorticography to pre‐cisely specify the range of epileptogenic focuses ,we performed resection of hippocampus and anterior temporal lobe.Then clini‐cal effect was collected and analyzed in postoperative follow-up period.Results During the intermittent period ,the habitual sei‐zures and epileptiform discharges were recorded in 20 cases.Epileptiform discharges were in unilateral temporal and hippocam‐pus regions in 14 cases ,of whom ,discharges also appearing in the homo-lateral frontal region occurred in 5 cases ;And epilepti‐form discharges were in bilateral temporal regions in 6 cases ,among whom 2 with discharges also appeared in the unilateral frontal region.Epileptiform discharges in 8 cases derived from left temporal lobe and hippocampus ,and 12 cases from the right.The examination of MEG showed that during the intervals of attacks ,20 cases had epileptiform discharges with the de‐tection rate of 100%.Of the 20 cases ,17 cases presented discharges in the unilateral temporal lobe among whom there were 8 occurring in the homo-lateral frontal region in addition;Furthermore ,3 cases displayed discharges in the bilateral temporal lobe.In the 24-month follow-up period ,sixteen cases got grade Ⅰ ,three got grade Ⅱ and one got grade Ⅲ.As a result ,the effect rate of operation was 100% with good surgery efficient rate of 95 %.Conclusion Surgical therapy helps to treat temporal lobe space-occupying lesions concomitant with epilepsy ,the multiple localization techniques of epileptogenic focuses can be jointly applied to bring operative effect up and reduce postoperative adverse events.关键词
